ADAM Questionairre

The Androgen Deficiency in Aging Males (ADAM) questionnaire is a helpful tool to assess if you may have low testosterone. The results of this questionnaire need to be confirmed with laboratory analysis.

We offer testosterone replacement therapy as:

  • Topical Cream: Sent to your home. Applied 1 -2 times per day.
  • Injections: Sent to your home, or administered in office. Injected 1 – 3 times per week.
  • Pellet: Lasts 3 to 6 months. In office only.
  • Sublingual Troche: Sent to your home. Administered under the tongue 2 – 3 times per day.
  • Enclomiphene/clomiphene: Daily oral medication. Best suited for younger men who wish to remain fertile.
  • HCG: As a monotherapy most men will inject HCG every other day to raise testosterone. Sent to a local pharmacy.

Some patients may also require a pharmaceutical to lower their estrogen levels called anastrozole.

In addition to testosterone replacement therapy, many of Dr. Smitherman’s patients also receive some combination of natural supplements, pharmaceutical medication, PRF injections, or shockwave therapy for erectile dysfunction. Most men will not require these additional measures when their testosterone, and estrogen are optimally managed, but for those who do, Dr. Smitherman will find the best option for you. Options include: sildenafil (generic Viagra), tadalafil (generic Cialis), vardenafil, Tri-mix, PRF injection, and shockwave therapy. These additional treatments are not covered under the testosterone replacement therapy program. Some prescriptions may be covered by your insurance.

Fertility can be maintained while on testosterone by taking hcg or clomiphene. For some younger patients therapeutic levels of testosterone can be attained without testosterone by taking hcg or clomiphene alone. On special occasions, Dr. Smitherman may recommend hcg to patients even when they are not interested in maintaining fertility. Dr. Smitherman tailors the testosterone replacement program to every patients individual needs.

    • Monthly Fee Testosterone Cream Program: $150 per month
    • Monthly Fee Testosterone Pellet Program: $175 per month
  • Office Visits:
    • Covered through monthly fee
    • Number of office visits will depend on medical necessity as determined by Dr. Smitherman.
    • Minimum 2 office visits per year is required for monitoring of patients on testosterone therapy.
    • Email, call, text, EHR messages included. I have no policy on limiting this communication, because everyone has always been reasonable in this regard.
  • Medications:
    • Testosterone (cream, injection, pellet), anastrozole, clomiphene covered through monthly fee.
    • Sent to your home.
    • Hcg not covered through monthly fee
    • Testosterone Troche not covered through monthly fee.
      • Cost: $100 – $125 per month.
  • Supplies:
    • Covered through monthly fee.
    • Send to your home.
  • Lab Testing: Covered through monthly fee. Testing is based on Dr. Smitherman’s clinical discretion. Typical testing is at 6 weeks, 18 weeks, 42 weeks of therapy. Then every 6-12 months after that. Initial labs are not covered by program.

The body is a complex orchestra of hormones. After your evaluation, Dr. Smitherman may recommend that you replace other hormones in addition to testosterone. These may include: dhea, pregnenolone, cortisol, thyroid hormone, and human growth hormone peptides. Dr. Smitherman also may suggest other pharmaceuticals, botanicals, or supplements outside of the testosterone therapy program. The goal is to help you live a healthy, and vibrant life. These additional therapies are not covered by testosterone replacement therapy program.

All pharmaceuticals come from FDA compliant pharmacies in the United States. Prescriptions are mailed directly to your home.
